Resurrecting this one. What was the issue with the card?

PAL for sync (hsync, vsync) was inserted backwards.

Unfortunately, UPS decided to play accordion with the box when it was sent back, resulting in the Picasso being bent quite a bit. It worked for a while, then failed. Tried to fix it, but the traces had been pulled from the board by the Cirrus Logic chip when the board was bent.

Oh no, that is terrible. Sorry to hear... I am always hesitant to ship old equipment like this, you never know what will happen in transit, no matter how well packaged it is.
